Convidose sachet for patients

ConviDose™ is a service that helps you arrange your medicine. The right amount of medicine are packed into each sachet according to the time you need to take them. If you have many long-term illnesses, it can be hard for you to remember if you have taken all the medicine. ConviDose™ is an easy-to-follow system which allows you as a patient or a caregiver to manage medicine easily and spot any missed doses.

Answer the following questions to find out if ConviDose™ is suitable for you:

Q1	Are you on four or more long-term medicine?
Q2	Do you have any problems in understanding medicine labels?
Q3	Do you have any problems in remembering when and how to take the medicine?
Q4	Do you find it difficult to take medicine out from medicine strips or bottles, or to break tablets into half?
Q5	Do you require the help of a caregiver to take care of their daily activities and medical needs?

If the answer is “Yes” to any of the above questions, ConviDose™ may be the solution for you. Currently, ConviDose™ service is available from the following NHG Polyclinics.
